When medical costs become high
Hospitalization, surgery and expensive treatment can result in large out-of-pocket payments. Understanding the high-cost medical expense benefit and annual caps can help you estimate costs.

Decision flow when expenses are high
Check the high-cost benefit and payment limit with your insurer
Check non-covered charges and meal fees separately
Three points to check first
Confirm your copayment rate and applicable benefits
Japan Health Insurance Association, health insurance society or National Health Insurance
Confirm the month of treatment and insurer procedures
Three important things to know
1. What is the high-cost medical expense benefit?
If your monthly insured medical copayment exceeds the ceiling based on age and income, the excess may be reimbursed. In some cases, expenses across multiple facilities or household members can be combined.
Costs depend on age, income group, treatment month, repeated-use reductions and aggregation rules. Confirm specific amounts and effective dates with your insurer or an official source.
2. What should I do?
Identify your insurer
Your employer’s health plan, Japan Health Insurance Association, health insurance society or National Health Insurance.
Check expected expenses
Ask the hospital about planned hospitalization, surgery or costly medicines.
Confirm the ceiling
Ask your insurer which income category and out-of-pocket limit apply.
Reimbursement and settlement
Apply as needed to receive reimbursement above the applicable ceiling.
3. Costs that may or may not qualify
| Expense | High-cost medical expense benefit |
|---|---|
| Insured consultations, tests, surgery and medicines | May qualify |
| Insured treatment while hospitalized | May qualify |
| Extra charges for a private room | Generally excluded |
| Meals during hospitalization | Generally excluded |
| Uninsured or cosmetic treatment | Excluded |
| Technical fees for advanced medical care | Generally excluded |
4. Long-term treatment
Important clarification:Before expensive treatment, speak to your insurer and the hospital billing desk. Ask whether the hospital can access payment-limit information through your My Number health insurance card, and if not, whether a certificate of eligibility for the limit is required. Check how to apply for refunds, required documents and deadlines. The source text states that annual caps (August through July) began in August 2026 and that income categories are scheduled to be subdivided in August 2027. Verify current amounts and implementation with the Ministry of Health and your insurer.
If the high-cost benefit applied in at least three of the previous twelve months, the monthly ceiling may be lower from the fourth month, under the repeated-use rule. The source text also notes an annual cap from August 2026.
Limits vary by income group, age, household, insurer and 2026–2027 rule changes. Confirm with your insurer before treatment.
